Violation Observed: Submit to the OOL, a copy of at least 2 staff's proof of current training in First Aid. 3A:52-5.3(o)(1) Physical plant requirements for all centers First aid requirements are as follows: at least two staff members who have current certified basic knowledge of first aid principles and cardiopulmonary resuscitation (CPR), as defined by a recognized health organization (such as the American Red Cross), shall be in the center at all times when enrolled children are present. The staff took temperatures, but did not ask about COVID-19 symptoms or testing. Violation Observed: Per the Updated COVID-19 Standards Child Care Centers, issued, ensure that the staff are screening the visitors prior to allowing them in the center. ![]() 3A:52-5.3(r) Physical plant requirements for all centers The center shall take any steps required by the Office of Licensing to correct conditions in the building or center that may endanger the health, safety, and well-being of the children served. Violation Observed: Submit to the OOL, a copy of the center's communicable disease policy. This policy shall include the following: the list of illnesses and symptoms of illness for which a child will be separated from the group and possibly sent home, as specified in N.J.A.C. 3A:52-7.11(a)(1) Information to parents regarding the management of communicable diseases Each center shall develop a written policy on the management of communicable diseases. Violation Observed: Submit to the OOL, a copy of the center's illness log for the previous 30 day period. ![]() 3A:52-7.9(a)(1) Illness log for early childhood programs For early childhood programs, the following shall apply: the center shall maintain on file a log of the initial illnesses, symptoms of illness, or diseases that are exhibited by each child while in the center’s care, as specified in N.J.A.C. Violation Observed: Label all bottles with the child's name and the current date. 3A:52-6.3(b)(3)(iv)(1) Food and nutrition For early childhood programs, the following shall apply, feeding requirements for centers serving children less than 18 months of age are as follows: for bottles, each child's bottle(s) shall be labeled with the child's name and dated. The sponsor or sponsor representative and each staff member shall complete the electronic fingerprinting process through the vendor authorized by the State to conduct CHRI background checks through the Division of State Police in the Department of Law and Public Safety and the Federal Bureau of Investigation. 3A:52-4.11(a)(1) Criminal History Record Information background check procedures As a condition of securing a license or Certificate of Life/Safety Approval, the sponsor or sponsor representative shall ensure that a Criminal History Record Information (CHRI) fingerprint background check is completed for himself or herself, and for all staff members at least 18 years of age who are or will be working at the center on a regularly- scheduled basis, to determine whether any such person has been convicted of a crime, as specified in P.L. A staff stated that there were 6 children, when 5 children were present. Violation Observed: Ensure that all staff know how many children are in their care. 3A:52-4.3(a)(2) Staff/child ratios and supervision The center shall ensure that all staff members are trained in the method of keeping track of children and know how many children are in their care at all times. The staff had to count the children when asked. A staff stated there were 13 children, when 14 were present. Violation Observed: Ensure that adequate procedures for tracking children are adhered to at all times throughout the center. 3A:52-4.3(a)(1) Staff/child ratios and supervision The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.
